This Receipt with federal and state appeal information is designed to be filled in and given to a customer at the point of sale by a dealer. It can also be used by a customer who is trying to complete a background check post September 13, 2023 at a dealer under the new NYS Police system. This is a PDF fillable, but can simply be printed and filled in by hand (if preferred).
